(一)教師資格認定申請表
1.申請人員網上填報完畢后用A4紙自行正反面打印(原件一式兩份,將思想品德鑒定表與申請表一起用膠水將左側粘貼整齊)。
2.思想品德鑒定表(一式兩份)按要求填好并蓋好公章。(戶口不在北京的人員申請認定教師資格,其思想品德鑒定,既要有工作單位的公章,還要有本市存放人事檔案機構(人才服務中心)的公章,沒有工作單位的本市申請人,其鑒定表上本人所在居委會蓋章后加蓋存檔機構的公章,有工作單位的本市申請人,單位有組織人事部門公章的鑒定表上蓋單位組織人事部門公章,單位無組織人事部門公章的鑒定表上蓋所在單位公章并加蓋檔案存放部門(人才服務中心)公章)。
(二)教育學、教育心理學成績(要求和條件)。

2012年陜西省中小學教師資格教育基礎理論知識考試成績已經公布,現就考試成績合格人員填報體檢表有關事宜通知如下:
一、成績查詢
2012年陜西省中小學教師資格教育基礎理論知識考試閱卷、登分工作由省教育廳組織實施,5月28日成績已向社會公布,請申請人員盡快登陸陜西省教育廳門戶網站(www.tmdps.cn/),按提示查詢本人考試成績并打印《陜西省2012年中小學教師資格教育基礎理論知識考試成績報告單》。
二、填報體檢表人員
申請高級中學、中等職業學校、初級中學、小學、幼兒園教師資格人員中:
1、報名時已審核認定教育基礎理論知識四科全部免試的人員;
2、參加了教育基礎理論知識考試,每科考試成績均在60分及以上的人員;
3、參加了2012年陜西省中小學教師資格教育基礎理論知識考試的應屆普通高校非師范教育類畢業生,每科考試成績均在60分及以上,且為陜西生源的考生。
三、辦理時間
6月4日(星期一)—8日(星期五)

3、教育學、心理學補修、考試報名
非師范教育類專業畢業以及雖為師范教育類專業畢業但與其擬申請教師資格種類師范類要求不符者,需按時參加教育學、心理學報名、補修、考試,并取得合格證書。申請購買考試用書、補修等具體事宜由各縣、區自考辦統一在申請人辦理考試報名手續時一并告知。
考試報名時間、地點:教育學、心理學報名時間為2008年12月8日-10日。具體報名工作由各縣區自學考試辦公室負責,考試工作由市考辦負責,考試時間為2009年2月28日。具體考試事宜由市自學考試辦公室根據省考試院要求另行通知。教育學、心理學合格證書由各縣、區自學考試辦公室負責發放。
報名提交材料 :本人戶口簿、有效身份證、有效學歷證書原件、二張正面免冠二寸照片(藍色背景),非連云港市戶籍的還需提供所在單位工作證明。
收費標準:按省物價局、省財政廳《關于教師資格考試收費標準及有關問題的通知》(蘇價費〔2007〕210號)執行,教育學、心理學每門報名考試費32元。
各縣區自學考試辦公室于2008年12月18日前將參加教育學、心理學考試的人員報名信息報市自學考試辦公室。
4、受理材料
2009年4月18日--19日符合教師資格認定條件的申請人上交申請材料,地點同報名地點。
受理時提交材料:
(1)《教師資格認定申請表》一式兩份,填表日期截止2009年4月19日前(含4月19日)
(2)有效身份證原件、復印件
(3)有效學歷證書(專科、本科)原件、復印件
(4)教育學、心理學合格證書原件、復印件
(5)教育教學基本素質及能力測試評價表
(6)普通話水平測試等級證書原件、復印件
(7)思想品德鑒定表
(8)體檢表(由教師資格認定機構統一投放材料袋)
(9)根據本次教師資格認定工作要求,申請者需提供電子版、紙質照片,電子版尺寸應在20K之內,像素為114(寬)×156(高),照片擴展名只能為.jpg,系統不接受其它格式的文件;紙質照片同電子版尺寸,背面注明姓名、單位。
5、資格認定
市、縣(區)教師資格認定機構根據職責權限,按照《江蘇省教師資格專家審查委員會組織辦法(試行)》的規定,組織教師資格專家審查委員會對申請人的材料進行綜合審查,并提出審查意見。
5月5日前,市、縣(區)教師資格認定機構按照《關于做好取得教師資格人員信息報送工作的通知》(蘇教師[2003]6號)要求,將代碼表、資格認定人員名冊、工作總結書面報市教育局政治處。
6、證書發放
2009年6月底前,對經審查符合教師資格認定條件者,頒發教師資格證書。
第五篇:對外漢語教師資格認證鄭重聲明
鄭重聲明
近來發現某些機構打著“國際漢語教師協會”的旗號在招生,說他們的漢語教師證書是得到認可的。經慎重調查,我們鄭重聲明如下:
一、經核實,“國際漢語教師協會”為子虛烏有。根據國家法律規定,任何行業協會的成立均須經過協會所在區域的國家民政局、工商局、公安局、稅務局的聯合批復方能成立。經查“國際漢語教師協會”均未得到以上行政部門注冊備案,故該組織為子虛烏有。
二、經調查,打著“國際漢語教師協會”名義,發漢語教師資格證書的機構有好幾家,但這幾家發的證書均不一樣,所認證的過程、考試的內容、考試的時間均不一樣。進步應正這個協會的子虛烏有,其發證行為純屬為個人行為,沒有任何法律效應,等同賣假證,屬于國家嚴厲打擊之范疇。
三、對外漢語教師的培訓屬于特殊教育行業,必須經過國家教育行政機關的特別審批方能展開培訓,只有取得相對應的辦學許可證方能開展相應培訓。公司性質的機構不得開展相應的教師培訓,但以上打著“國際漢語教師協會”招生的機構均為公司性質,不具備任何辦學資質。
四、“IPA國際注冊漢語教師資格證”發證單位為“IPA國際認證協會”,是經過國家行政部門審批設立的,在國家工商總局、民政部門、公安局均有注冊備案,其備案號為:***。
五、目前只有“IPA國際注冊漢語教師資格證”是得到國家人事部唯一授權認可并可在全國進行考試的;也只有IPA國際注冊漢語教師資格證書才能在國家人事部的官方網站上查詢及注冊備案。
六、目前也只有IPA國際注冊漢語教師資格證書在全國實行了統一考試,唯有該考試才有國家人事部審定的指定考試大綱、考試指南、考試用書,并且在全國各個主要城市均有考點。其他某些機構稱謂的漢語教師資格考試,既無相關國家權力部門給予的授權,也無相應考試大綱、考試指南及考試用書,更無遍及全國的考點。既不合法更無權威可言。
七、“京師環宇”以北京師范大學為依托,是經過國家教委特殊審批的具有對外漢語教師培訓資質的正規辦學機構,其辦學許可證號為:京教***。上課均在與當地合作的國家重點院校內。如北京師范大學、中國傳媒大學、華中師范大學、云南大學、沈陽師范大學、黑龍江大學、合肥師范學院、太原師范學院等。
