本人___________（姓名及職務(wù)）為進(jìn)口貨物收貨人/進(jìn)口貨物收貨人代理人（不適用的部分請(qǐng)劃去），茲聲明編號(hào)為_(kāi)___________的報(bào)關(guān)單所列第______項(xiàng)貨物原產(chǎn)自新加坡, 且貨物符合《中華人民共和國(guó)政府和新加坡共和國(guó)政府自由貿(mào)易協(xié)定》原產(chǎn)地規(guī)則的要求。

本人申請(qǐng)對(duì)上述貨物適用《中華人民共和國(guó)政府和新加坡共和國(guó)政府自由貿(mào)易協(xié)定》協(xié)定稅率，并申請(qǐng)繳納保證金后放行貨物。本人承諾在自貨物進(jìn)口之日起1年內(nèi)補(bǔ)交《中華人民共和國(guó)政府和新加坡共和國(guó)政府自由貿(mào)易協(xié)定》原產(chǎn)地證書(shū)。

* A Certificate of Origin under China-Chile Free Trade Agreement shall be valid for one year from the date of issue in the exporting country.4Overleaf Instruction

Box 1:

State the full legal name, address(including country)of the exporter.Box 2:State the full legal name, address(including country)of the producer.If more than one producer’s good is included in the certificate, list the additional producers, including name, address(including country).If the exporter or the producer wishes the information to be confidential, it is acceptable to state “Available to the competent governmental authority upon request”.If the producer and the exporter are the same, please complete field with “SAME”.If the producer is unknown, it is acceptable to state “UNKNOWN”.Box 3:State the full legal name, address(including country)of the consignee.Box 4:Complete the means of transport and route and specify the departure date, transport vehicle No., port of loading and discharge.Box 5:The customs authorities of the importing country must indicate(√)in the relevant boxes whether or not preferential tariff treatment is accorded.Box 6:Customer’s Order Number, Letter of Credit Number, and etc.may be included if required.If the invoice is issued by a non-Party operator, the name, address of the producer in the originating Party shall be stated herein.Box 7:

中國(guó)-智利自貿(mào)區(qū)協(xié)定項(xiàng)下的原產(chǎn)地證書(shū)應(yīng)在自出口方簽發(fā)之日起一年內(nèi)有效。

6背面說(shuō)明

第一欄：應(yīng)填寫(xiě)出口商詳細(xì)的依法登記的名稱、地址（包括國(guó)家）。

第二欄：在已知的情況下填寫(xiě)生產(chǎn)商詳細(xì)的依法登記的名稱、地址（包括國(guó)家）。如果證書(shū)包含一

個(gè)以上生產(chǎn)商的商品，應(yīng)該列出其他生產(chǎn)商的詳細(xì)名稱、地址（包括國(guó)家）。如果出口商或生產(chǎn)商希望對(duì)信息予以保密，可以填寫(xiě)“應(yīng)要求提供給簽證機(jī)構(gòu)（Available to competent governmental authorityupon request）”。如果生產(chǎn)商和出口商相同，應(yīng)填寫(xiě)“相同（SAME）”。如果不知道生產(chǎn)商，可填寫(xiě)“不知道（UNKNOWN）”。

第三欄：應(yīng)填寫(xiě)收貨人詳細(xì)的依法登記的名稱、地址（包括國(guó)家）。

第四欄：應(yīng)據(jù)所知填寫(xiě)運(yùn)輸方式及路線、詳細(xì)說(shuō)明離港日期、運(yùn)輸交通工具的編號(hào)、裝貨口岸和

到貨口岸。

第五欄：不論是否給予優(yōu)惠待遇，進(jìn)口方海關(guān)必須在相應(yīng)欄目標(biāo)注(√)。

第六欄：如有要求可以填寫(xiě)顧客順序號(hào)、信用證號(hào)等。如果發(fā)票是由非締約方開(kāi)出的，應(yīng)在此欄

標(biāo)注原產(chǎn)國(guó)生產(chǎn)商的名稱、地址和國(guó)家。

第七欄：應(yīng)填寫(xiě)項(xiàng)目號(hào)，但不得超過(guò)20項(xiàng)。第八欄：應(yīng)填寫(xiě)嘜頭及包裝號(hào)。

第九欄：應(yīng)詳細(xì)列明包裝數(shù)量及種類。對(duì)每種貨物提供詳細(xì)的貨物描述，以便于查驗(yàn)的海關(guān)關(guān)員

可以識(shí)別。貨物描述應(yīng)與發(fā)票描述及貨物的協(xié)調(diào)制度編碼相符。如果是散裝貨，應(yīng)注明“散裝”。當(dāng)貨物描述結(jié)束時(shí)，加上“***”（三顆星）或“＼”（結(jié)束斜線符號(hào)）。

第十欄：應(yīng)對(duì)應(yīng)第九欄中的每種貨物填寫(xiě)協(xié)調(diào)制度編碼，以六位編碼為準(zhǔn)。

第十一欄：若貨物符合原產(chǎn)地規(guī)則，出口商必須按照下列表格中規(guī)定的格式，在本證書(shū)第十一欄

量。

第十三欄：應(yīng)填寫(xiě)發(fā)票號(hào)、發(fā)票日期及發(fā)票價(jià)。

第十四欄：本欄必須由出口商填寫(xiě)、簽名并填寫(xiě)日期。且應(yīng)該填寫(xiě)簽名的地點(diǎn)及日期。

第十五欄：本欄必須由簽證機(jī)構(gòu)的授權(quán)人員填制、簽名、填寫(xiě)簽證日期并蓋章。并應(yīng)提供簽證機(jī)構(gòu)的電話號(hào)碼，傳真及地址。

第三篇：原產(chǎn)地證書(shū)

（2）in reimbursement draw your own sight drafts in sterling on…Bank and forward them to our London Office, accompanied by your certificate that all terms of this letter of credit have been complied with 償付辦法，由你行開(kāi)出英鎊即期匯票向……銀行支取。在寄送匯票給我倫敦辦事處時(shí)，應(yīng)隨附你行的證明，聲明本證的全部條款已經(jīng)履行

（3）available by your draft at sight payable by us in London on the basis to sight draft on New York 憑你行開(kāi)具之即期匯票向我行在倫敦的機(jī)構(gòu)索回票款，票款在紐約即期兌付

（4）in reimbursement, please claim from our RMB￥ account held with your banking department Bank of China Head Office Beijing with the amount of your negotiation 償付辦法，請(qǐng)?jiān)诒本┛傂形胰嗣駧艓糁兴骰啬阈凶h付之款項(xiàng)

C/O為CERTIFICATE OF ORIGIN的簡(jiǎn)稱，意思為“原產(chǎn)地證書(shū)”，以下為具體內(nèi)容。

1作用編輯

原產(chǎn)地證明書(shū)（CERTIFICATE OF ORIGIN）是出口商應(yīng)進(jìn)口商要求而提供的、由公證機(jī)構(gòu)或政府或出口商出具的證明貨物原產(chǎn)地或制造地的一種證明文件。原產(chǎn)地證書(shū)是貿(mào)易關(guān)系人交接貨物、結(jié)算貨款、索賠理賠、進(jìn)口國(guó)通關(guān)驗(yàn)收、征收關(guān)稅的有效憑證，它還是出口國(guó)享受配額待遇、進(jìn)口國(guó)對(duì)不同出口國(guó)實(shí)行不同貿(mào)易政策的憑證。2類型編輯

根據(jù)簽發(fā)者不同，原產(chǎn)地證書(shū)一般可分為以下三類： ①商檢機(jī)構(gòu)出具的原產(chǎn)地證書(shū)，如：中華人民共和國(guó)檢驗(yàn)檢疫局（CIQ）出具的普惠制產(chǎn)地證格式A（GSP FORM A）；一般原產(chǎn)地證書(shū)（CERTIFICATE OF ORIGIN）。

②商會(huì)出具的產(chǎn)地證書(shū)，如：中國(guó)國(guó)際貿(mào)易促進(jìn)委員會(huì)（CCPIT）出具的一般原產(chǎn)地證書(shū)，簡(jiǎn)稱貿(mào)促會(huì)產(chǎn)地證書(shū)（CCPIT CEERTIFICATE OF ORIGIN）；

③制造商或出口商出具的產(chǎn)地證書(shū)；

在國(guó)際貿(mào)易實(shí)務(wù)中，應(yīng)該提供哪種產(chǎn)地證明書(shū)，主要依據(jù)合同或信用證的要求。一般對(duì)于實(shí)行普惠制國(guó)家出口貨物，都要求出具普惠制產(chǎn)地證明書(shū)。如果信用證并未明確規(guī)定產(chǎn)地證書(shū)的出具者，那么銀行應(yīng)該接受任何一種產(chǎn)地證明書(shū)。現(xiàn)在我國(guó)多數(shù)出口商習(xí)慣于使用貿(mào)促會(huì)出具的證書(shū)。3申請(qǐng)編輯

根據(jù)我國(guó)的規(guī)定，企業(yè)最遲于貨物報(bào)關(guān)出運(yùn)前三天向簽證機(jī)構(gòu)申請(qǐng)辦理原產(chǎn)地證書(shū)，并嚴(yán)格按照簽證機(jī)構(gòu)的要求，真實(shí)、完整、正確地填寫(xiě)以下材料：

①《中華人民共和國(guó)出口貨物原產(chǎn)地證書(shū)/加工裝配證明書(shū)申請(qǐng)書(shū)》；

（見(jiàn)附頁(yè)）此處顯示一份《一般原產(chǎn)地證明書(shū)/加工裝配證明書(shū)》圖5-1（國(guó)際商務(wù)單證103）

②《中華人民共和國(guó)出口貨物原產(chǎn)地證明書(shū)》一式四份； ③出口貨物商業(yè)發(fā)票；

④簽證機(jī)構(gòu)認(rèn)為必要的其他證明文件。

4主要內(nèi)容編輯

⑴ 進(jìn)出口商的名稱及地址； ⑵運(yùn)輸方式及航線； ⑶商品嘜頭和編號(hào)； ⑷商品名稱、數(shù)量和重量； ⑸證明文字等。

5填寫(xiě)編輯 ⑴產(chǎn)地證書(shū)的編號(hào)（CERTIFICATE NO。）此欄不得留空，否則證書(shū)無(wú)效。⑵出口方（EXPORTER）

填寫(xiě)出口公司的詳細(xì)地址、名稱和國(guó)家（地區(qū)）名。若經(jīng)其他國(guó)家或地區(qū)，需填寫(xiě)轉(zhuǎn)口商名稱時(shí)，可在出口商后面填英文VIA，然后再填寫(xiě)轉(zhuǎn)口商名稱、地址和國(guó)家。

⑶收貨方（CONSIGNEE）

填寫(xiě)最終收貨人名稱、地址和國(guó)家（地區(qū)）名。通常是外貿(mào)合同中的買(mǎi)方或信用證上規(guī)定的提單通知人。如信用證規(guī)定所有單證收貨人一欄留空，在這種情況下，此欄應(yīng)加注“TO WHOM IT MAY CONCERN”或“TO ORDER”，但此欄不得留空。若需填寫(xiě)轉(zhuǎn)口商名稱時(shí)，可在收貨人后面加填英文VIA，然后再寫(xiě)轉(zhuǎn)口商名稱、地址、國(guó)家。

⑷運(yùn)輸方式和路線（MEANS OF TRANSPORT AND ROUTE）填寫(xiě)裝運(yùn)港和目的港、運(yùn)輸方式。若經(jīng)轉(zhuǎn)運(yùn)，還應(yīng)注明轉(zhuǎn)運(yùn)地。例如：通過(guò)海運(yùn)，由上海港經(jīng)香港轉(zhuǎn)運(yùn)至漢堡港，應(yīng)填為： FROM SHANGHAI TO HAMBURG BY VESSEL VIA HONGKONG。

⑸目的地國(guó)家（地區(qū)）（COUNTRY/REGION OF DESTINATION）

填寫(xiě)目的地國(guó)家（地區(qū)）。一般應(yīng)與最終收貨人或最終目的港（地）國(guó)別相一致，不能填寫(xiě)中間商國(guó)家名稱。

⑹簽證機(jī)構(gòu)用欄（FOR CERTIFYING AUTHORITY USE ONLY）由簽證機(jī)構(gòu)在簽發(fā)后發(fā)證、補(bǔ)發(fā)證書(shū)或加注其他聲明時(shí)使用。證書(shū)申領(lǐng)單位應(yīng)將此欄留空。一般情況下，該欄不填。

⑺運(yùn)輸標(biāo)志（MARKS AND NUMBERS）填寫(xiě)嘜頭。應(yīng)按信用證、合同及發(fā)票上所列嘜頭填寫(xiě)完整圖案、文字標(biāo)記及包裝號(hào)碼，不可簡(jiǎn)單填寫(xiě)“按照發(fā)票”（AS PER INVOICE NUMBER）或者“按照提單”（AS PEER B/L NUMBER）。貨物如無(wú)嘜頭，應(yīng)填寫(xiě)“無(wú)嘜頭”（NO MARK）字樣。此欄不得空留，如嘜頭多，本欄目填寫(xiě)不夠，可填寫(xiě)在第7、8、9欄內(nèi)的空白處，如還是不夠，可用附頁(yè)填寫(xiě)。

⑻商品描述、包裝數(shù)量及種類（NUMBER AND KIND OF PACKAGES；DESCRIPTION OF GOODS）

填寫(xiě)商品描述及包裝數(shù)量。商品名稱要填寫(xiě)具體名稱，不得用概括性表述，例如服裝、食品（GARMENT、FOOD）等。包裝數(shù)量及種類要按具體單位填寫(xiě)，應(yīng)與信用證及其他單據(jù)嚴(yán)格一致。包裝數(shù)量應(yīng)在阿拉伯?dāng)?shù)字后加注英文表述。如貨物為散裝，在商品名稱后加注“散裝”（IN BULK）字樣。有時(shí)信用證要求在所有單據(jù)上加注合同號(hào)碼、信用證號(hào)等，可加注在此欄內(nèi)。本欄的末行要打上表示結(jié)束的符號(hào)（**************），以防加添內(nèi)容。

⑼商品編碼（HS CODE）

此欄要求填寫(xiě)HS編碼，應(yīng)與報(bào)關(guān)單一致。若同一證書(shū)包含有幾種商品，則應(yīng)將相應(yīng)的稅目號(hào)全部填寫(xiě)。此欄不得空留。

⑽數(shù)量（QUANTITY）

此欄要求填寫(xiě)出口貨物的數(shù)量及商品的計(jì)量單位。如果只有毛重時(shí)，則需填“G.W.”。

⑾發(fā)票號(hào)碼及日期（NUMBER AND DATE OF INVOICE）填寫(xiě)商業(yè)發(fā)票號(hào)碼及日期。此欄不得空留，為避免對(duì)月份、日期的誤解，月份一律用英文表述，如2004年3月15日，則為： MARCH.15，2004。

⑿出口方聲明（DECLARATION BY THE EXPORTER）填寫(xiě)出口人的名稱、申報(bào)地點(diǎn)及日期，由已在簽證機(jī)構(gòu)注冊(cè)的人員簽名并加蓋有中英文的印章。

⒀由簽證機(jī)構(gòu)簽字、蓋章（CERTIFICATION）

填寫(xiě)簽證地點(diǎn)、日期。簽證機(jī)構(gòu)簽證人經(jīng)審核后在此欄（正本）簽名，并蓋簽證印章。6更改重發(fā)編輯

對(duì)簽證機(jī)構(gòu)已簽發(fā)的原產(chǎn)地證書(shū)，當(dāng)申請(qǐng)單位需要更改其內(nèi)容時(shí)，申請(qǐng)單位應(yīng)書(shū)面申明理由，提交已更改的產(chǎn)地證，并退回原證書(shū)正本。

對(duì)簽證機(jī)構(gòu)已簽發(fā)的原產(chǎn)地證明書(shū)遺失或損毀，申請(qǐng)單位應(yīng)書(shū)面說(shuō)明遺失或損毀的原因，提交重新填制的產(chǎn)地證書(shū)副本或復(fù)印件

原產(chǎn)地證書(shū)的分類

原產(chǎn)地證書(shū)主要分為三類：一般原產(chǎn)地證、普惠制原產(chǎn)地證和區(qū)域性優(yōu)惠原產(chǎn)地證。

1.一般原產(chǎn)地證（簡(jiǎn)稱C.O.證書(shū)）是證明貨物原產(chǎn)于某一特定國(guó)家和地區(qū)、享受進(jìn)口國(guó)正常關(guān)稅（最惠國(guó)）待遇的證明文件，是進(jìn)口國(guó)對(duì)進(jìn)口貨物實(shí)施管理和征稅，準(zhǔn)予放行，實(shí)施數(shù)量控制和反傾銷(xiāo)等措施的依據(jù)。

2.普惠制是發(fā)達(dá)國(guó)家給予發(fā)展中國(guó)家出口制成品和半制成品(包括某些初級(jí)產(chǎn)品)普遍的、非歧視的、非互惠的一種關(guān)稅優(yōu)惠制度。普惠制原產(chǎn)地證（簡(jiǎn)稱FORM A 證書(shū)）是受惠國(guó)貨物出口到給惠國(guó)時(shí)享受普惠制關(guān)稅優(yōu)惠待遇的官方憑證。

3.區(qū)域性優(yōu)惠原產(chǎn)地證是訂有區(qū)域性優(yōu)惠貿(mào)易協(xié)定國(guó)家官方機(jī)構(gòu)簽發(fā)的享受成員國(guó)關(guān)稅減免待遇的憑證。目前我國(guó)與不同國(guó)家簽訂的自由貿(mào)易協(xié)定對(duì)持有區(qū)域性優(yōu)惠原產(chǎn)地證書(shū)的貨物進(jìn)行大幅度的關(guān)稅減免，大部分產(chǎn)品都將在協(xié)定生效后幾年內(nèi)陸續(xù)享受零關(guān)稅。企業(yè)對(duì)出口貨物辦理不同原產(chǎn)地證書(shū)在進(jìn)口國(guó)海關(guān)享受的關(guān)稅待遇也不相同。一般來(lái)說(shuō)，一般原產(chǎn)地證書(shū)關(guān)稅優(yōu)惠幅度最少，普惠制和區(qū)域性證書(shū)的關(guān)稅優(yōu)惠幅度較大。

各類原產(chǎn)地證書(shū)簡(jiǎn)介

下面是各種原產(chǎn)地證書(shū)簽發(fā)所涉及的國(guó)家和地區(qū)：

亞太貿(mào)易協(xié)定：自2002年10月1日起簽發(fā)起，關(guān)稅減讓涉及2000多個(gè)產(chǎn)品。憑借亞太證書(shū)，出口到韓國(guó)的充電電池（HS編碼為850720）能享受韓方給予的30%的降稅幅度，調(diào)節(jié)閥（HS編碼為848180）能享受50%的降稅幅度，女士服裝的關(guān)稅稅率也從13%降到

8.1%；出口到印度的水產(chǎn)品的稅率從最惠國(guó)稅率30%直接降到零。

中國(guó)-東盟自由貿(mào)易區(qū)：自2004年1月1日起簽發(fā)起，降稅計(jì)劃涉及7000多個(gè)產(chǎn)品，到2010年1月1日大部分產(chǎn)品零關(guān)稅。以到泰國(guó)的商品為例，我區(qū)出口量較大的小家電及木家具最惠國(guó)稅率為20%，憑借Form E原產(chǎn)地證書(shū)，關(guān)稅逐年遞減，直到2010年享受零關(guān)稅。

中國(guó)-智利自由貿(mào)易區(qū)：自2006年10月1日起簽發(fā)，目前5891種產(chǎn)品已為關(guān)稅為零，97%的產(chǎn)品10年內(nèi)分階段降為零。憑借中智原產(chǎn)地證書(shū)，龍崗區(qū)到智利出口量較大的的空調(diào)和鞋類分別從20%和24%稅率降為零關(guān)稅。

中國(guó)-巴基斯坦自由貿(mào)易區(qū)：自2006年1月1日起簽發(fā)，2007年7月1日第一次降稅，2010年7月1日前36%的產(chǎn)品關(guān)稅為零。憑借中巴原產(chǎn)地證書(shū)，出口量大的玩具和空調(diào)分別能享受巴基斯坦給予的12%和5%的降稅幅度。

中國(guó)-新西蘭自由貿(mào)易區(qū)：自2008年10月1日起簽發(fā)，目前63.6%產(chǎn)品已降為零關(guān)稅,2016年1月1日前取消全部關(guān)稅。中新自由貿(mào)易區(qū)采取分步逐漸降稅的方式，如：龍崗區(qū)出口新西蘭貨值較大服裝，基準(zhǔn)稅率為7%，2009年降為4.2%，2012年實(shí)現(xiàn)零關(guān)稅；蓄電池基準(zhǔn)稅率為19%，2009年降為14.8%，2016年實(shí)現(xiàn)零關(guān)稅。

中國(guó)—新加坡自由貿(mào)易區(qū)：自 2009年1月1日起簽發(fā)，目前對(duì)來(lái)自中國(guó)除啤酒等個(gè)別產(chǎn)品外實(shí)行零關(guān)稅。

中國(guó)—秘魯自由貿(mào)易區(qū)：中秘雙方將對(duì)各自90%以上的產(chǎn)品分階段實(shí)施零關(guān)稅。其中龍崗區(qū)出口量較大的冰鮮魚(yú)、燈具、塑膠制品、電話、繼電器、馬達(dá)、鐘表等立刻享受零關(guān)稅進(jìn)入秘魯，玩具將在5年內(nèi)關(guān)稅降為零，各類服裝、充電電池和家具也會(huì)在5至10年內(nèi)逐步降為零。
