第一篇：一般原產地證書-CO

一般原產地證書：各國根據各自的原產地規則簽發的，證明貨物原產于某一特定國家或地區，享受進口國正常關稅待遇的證明文件。

中華人民共和國出口貨物原產地證書采用全國統一的證書格式，一正三副。證書用英文填制。

填制規范如下：

第一欄：出口商，地址和國家

填寫出口方的名稱，詳細地址，以及國家，此欄不的留空。若經其他國家中轉或地區則需要填寫轉運商名稱，一般在出口商英文后面加上VIA，例如：HANGZHOU TRUEMAX MACHINERY AND EQUIPMENT CO,.LTD.NO.182 GUODU DEVELOPMENT ,CHAOHUI RODE

第二欄：收貨人名稱，地址，國家。

填寫最終收貨方名稱，詳細地址，以及國家。

當信用證規定留空時，此欄加注“ TO WHOMIT CONTERN ”或者“TO ORDER”如果有轉口商名稱時。則在收貨人后面加上VIA，再填寫轉口商名稱，地址以及國家。

第三欄：運輸方式及路線

應填寫從裝貨港一直到目的港口的詳細的運輸路線，如經轉運，應注明轉運地。

第四欄：目的地國家/地區

應該填寫貨物最終運抵港口，一般與最終收貨人或最終目的港國別不一致，不得填寫中間商國別。不理解暫時

第五欄;簽證機構使用（杭州貿促會/浙江貿促會）

正常情況下是空白的，特殊情況下：

1：貨物已經出口，簽證日期遲于出貨日期，簽發“補發”證書時，此欄顯示：“ISSUED RETROSPECTIVELY”字樣。

2：證書有效期1年內申請補發。

3：證書遺失被盜或者損毀，簽發副本時，此欄顯示“DUPLICATE”。同時在此欄注明原證書編號和簽證日期，同時聲明原證書作廢，文字是“THIS CERTIFICATE IS IN REPLACEMENT OF CERTIFICATE OF ORIGIN NO ~~~~DATE~~~WHICH CANCELLED”.第六欄：嘜頭及包裝號

嘜頭需與發票上嘜頭一致，無嘜頭則填寫N/M,如果嘜頭較多情況下，可以填寫在7.8.9欄空白處或者添加附頁，此欄不得留空。

第七欄：包裝數量以及種類，商品名稱

不能籠統寫PACKAGE,如果是散裝則在寫成PACKAGE(INBULK)，如是裸裝寫成PACKAGE(NUDE CARGO)。

商品名稱也必須具體寫明，不能只寫成，機器（MACHINE）,服裝（GARMENT）等。

第八欄：商品編碼

此欄要求填寫商品的H.S品目號，若同一份證書包含幾種商品，則應該將相應的H.S品目號全部填寫，此欄不得留空。

第九欄：毛重或者其他數量

此欄需填寫正常商品計量單位，如“只”“件”“打”等。

貨物以重量計的填寫毛重，只有凈重的填寫凈重，但是需要注明，N.W 例如：N.W,112KGS

第十欄：發票號碼及日期

月份一律用英文填寫；

日期必須和發票號碼上一致。

第十一欄：出口商申明

此欄填打申報地點，日期，例如我司：HANGZHOU,CHINA,JUL.28.2011

申報單位應授權專人在此欄手簽（手簽員證書發下之后，我司曾曉蓉簽字，僅適用于杭州貿促會）

并加蓋單位中英文印章。（我公司中英文條形章）

第十二欄：簽證機構證明

由簽證機構簽字蓋章，并填打簽證地點，日期。

簽證日期不的早于申報日期和發票日期。

一般原產地證書一般使用英文，如果信用證有特殊要求使用其他文種的也可以接受。

第二篇：一般原產地證書CO的樣本

一般原產地證書

CO的類別

一般原產地證明書（C.O.）可以分為兩種，一種是由中國國際貿易促進委員會（簡稱CCPIT）簽發，另外一種是由中國進出口檢驗檢疫中心（簡稱CIQ）簽發。其中 CCPIT是可以代表的機構，所以國外進口商要求出口方出具由中國商會簽發的CO時，可以去貿促會加蓋“CCPIT代表中國商會”的章。

CO的作用

C.O.產地證是用以證明有關出口貨物和制造地的一種證明文件，是貨物在國際貿易行為中的“原籍”證書。在特定情況下進口國據此對進口貨物給予不同的關稅待遇。在國際貿易中，世界各國根據各自的對外貿易政策，普遍實行進口貿易管制，對進口商品實施差別關稅和數量限制，并由海關執行統計。進口國要求出口國出具貨物的原產地證明，已成為國際慣例，因此C.O.產地證是進行國際貿易的一項重要證明文件，歸納起來，具有以下幾方面的作用：

確定產品關稅待遇提高市場競爭力的重要工具

C.O.產地證則是各國海關據以征收關稅和實施差別待遇的有效憑證。如在進口國與出口國的政府之間定有關稅協定，用條約形式規定了協定稅率（AGREED CUSTOMS RATE），或兩國之間條約上規定了最惠國條款（MOST FAVORED NATION CLAUSE），買方往往要求賣方提供有效的產地證明書來證明進口貨物的原產地確系締約的對方國才能獲得相應的稅率待遇。

證明商品內在品質提高商品競爭力的作用

如在國際市場上持有中國原產地證的絲綢比持有其他不產絲綢國家產地證的絲綢更能賣好價。此外，產地證有時還是貿易雙方進行交接、結匯的必備單據。如買方在申請開信用證（L/C）時常要求提供C.O.產地證以確保其自身利益，銀行也常以C.O.產地證作為信用證（L/C）是否解付的重要憑證。

對進出口貨物進行統計的職責

原產地證則是海關籍以對進口貨物進行統計的重要依據。

進行貿易管理的工具

世界各國根據其貿易政策，為保護本國工業生產和國際貿易競爭需要，往往對某些貨物實行限制，制定一些進口貨物數量控制措施，例如：進口配額、許可證制度、反傾銷、反補貼制度。為實行這些控制制度，首先需確定進口的貨物是來自哪個國家，然后確定這批貨物是否受到進口數量限制，是否需持有進口許可證，是否要沖銷配額及征收反傾銷、反補貼稅……產地證也就成為實施這些制度的重要工具。

* A Certificate of Origin under China-Chile Free Trade Agreement shall be valid for one year from the date of issue in the exporting country.4Overleaf Instruction

Box 1:

State the full legal name, address(including country)of the exporter.Box 2:State the full legal name, address(including country)of the producer.If more than one producer’s good is included in the certificate, list the additional producers, including name, address(including country).If the exporter or the producer wishes the information to be confidential, it is acceptable to state “Available to the competent governmental authority upon request”.If the producer and the exporter are the same, please complete field with “SAME”.If the producer is unknown, it is acceptable to state “UNKNOWN”.Box 3:State the full legal name, address(including country)of the consignee.Box 4:Complete the means of transport and route and specify the departure date, transport vehicle No., port of loading and discharge.Box 5:The customs authorities of the importing country must indicate(√)in the relevant boxes whether or not preferential tariff treatment is accorded.Box 6:Customer’s Order Number, Letter of Credit Number, and etc.may be included if required.If the invoice is issued by a non-Party operator, the name, address of the producer in the originating Party shall be stated herein.Box 7:

中國-智利自貿區協定項下的原產地證書應在自出口方簽發之日起一年內有效。

6背面說明

第一欄：應填寫出口商詳細的依法登記的名稱、地址（包括國家）。

第二欄：在已知的情況下填寫生產商詳細的依法登記的名稱、地址（包括國家）。如果證書包含一

個以上生產商的商品，應該列出其他生產商的詳細名稱、地址（包括國家）。如果出口商或生產商希望對信息予以保密，可以填寫“應要求提供給簽證機構（Available to competent governmental authorityupon request）”。如果生產商和出口商相同，應填寫“相同（SAME）”。如果不知道生產商，可填寫“不知道（UNKNOWN）”。

第三欄：應填寫收貨人詳細的依法登記的名稱、地址（包括國家）。

第四欄：應據所知填寫運輸方式及路線、詳細說明離港日期、運輸交通工具的編號、裝貨口岸和

到貨口岸。

第五欄：不論是否給予優惠待遇，進口方海關必須在相應欄目標注(√)。

第六欄：如有要求可以填寫顧客順序號、信用證號等。如果發票是由非締約方開出的，應在此欄

標注原產國生產商的名稱、地址和國家。

第七欄：應填寫項目號，但不得超過20項。第八欄：應填寫嘜頭及包裝號。

第九欄：應詳細列明包裝數量及種類。對每種貨物提供詳細的貨物描述，以便于查驗的海關關員

可以識別。貨物描述應與發票描述及貨物的協調制度編碼相符。如果是散裝貨，應注明“散裝”。當貨物描述結束時，加上“***”（三顆星）或“＼”（結束斜線符號）。

第十欄：應對應第九欄中的每種貨物填寫協調制度編碼，以六位編碼為準。

第十一欄：若貨物符合原產地規則，出口商必須按照下列表格中規定的格式，在本證書第十一欄

量。

第十三欄：應填寫發票號、發票日期及發票價。

第十四欄：本欄必須由出口商填寫、簽名并填寫日期。且應該填寫簽名的地點及日期。

第十五欄：本欄必須由簽證機構的授權人員填制、簽名、填寫簽證日期并蓋章。并應提供簽證機構的電話號碼，傳真及地址。

第五篇：原產地證書

2．method of reimbursement 索償辦法

（1）in reimbursement, we shall authorize your Beijing Bank of China Head Office to debit our Head Office RMB Yuan account with them, upon receipt of relative documents 償付辦法，我行收到有關單據后，將授權你北京總行借記我總行在該行開立的人民幣帳戶

（2）in reimbursement draw your own sight drafts in sterling on…Bank and forward them to our London Office, accompanied by your certificate that all terms of this letter of credit have been complied with 償付辦法，由你行開出英鎊即期匯票向……銀行支取。在寄送匯票給我倫敦辦事處時，應隨附你行的證明，聲明本證的全部條款已經履行

（3）available by your draft at sight payable by us in London on the basis to sight draft on New York 憑你行開具之即期匯票向我行在倫敦的機構索回票款，票款在紐約即期兌付

（4）in reimbursement, please claim from our RMB￥ account held with your banking department Bank of China Head Office Beijing with the amount of your negotiation 償付辦法，請在北京總行我人民幣帳戶中索回你行議付之款項

（5）upon presentation of the documents to us, we shall authorize your head office backing department by airmail to debit the proceeds to our foreign business department account 一俟向我行提交單證，我行將用航郵授權你總行借記我行國外營業部帳戶

（6）after negotiation, you may reimburse yourselves by debiting our RMB￥ account with you, please forward all relative documents in one lot to us by airmail 議付后請借記我行在你行開立的人民幣帳戶，并將全部有關單據用航郵一次寄給我行

（7）all bank charges outside U.K.are for our principals account, but must claimed at the time of presentation of documents 在英國境外發生的所有銀行費用，應由開證人負擔，但須在提交單據是索取 8）negotiating bank may claim reimbursement by T.T.on the…bank certifying that the credit terms have been complied with 議付行須證明本證條款已旅行，并按電匯條款向……銀行索回貨款 9）negotiating bank are authorized to reimburse themselves to amount of their negotiation by redrawing by airmail at sight on…bank attaching to the reimbursement draft their certificate stating that all terms of the credit have been complied with and that the original and duplicate drafts and documents have been forwarded to us by consecutive airmail 議付行用航郵向…銀行重開一份即期匯票索取議付條款。索償匯票須附上證明，聲明本證所有條款已履行，單據的正副本已由連續航次寄交我行。
