三、參加競賽辦法

4.高一年級各組有預賽、決賽的徑賽項目,預賽取前十二名,其中前八名進入決賽,后

四名按相應名次記分;高一年級各組有預決賽的田賽項目預賽后均取前十二名進入決賽。

8.凡大會收到有班主任、班長簽名的信,檢舉某運動員冒名頂替參加比賽,如經核實為作弊行為,總裁判長簽字。則取消該運動員的所得名次,再扣該班團體總分9分。如冒名頂替參加比賽的運動員未得分,仍扣該班團體總分9分。班出現運動員冒名頂替等作弊行為,則取消該班參加“文明”班的評比。

五、獎勵辦法

1.單項、團體獲名次,都分別給予物質或精神獎勵。

2.破校紀錄、年級紀錄給相應的破紀錄獎。

六、報名方法

1、電腦報名時間安排:高三10月10日(星期一)第九節課,高二10月12日(星期三)

第八節課,鳳起校區高一10月11日(星期二)第八、九節課。要求各班男女體育委員、電腦操作快的同學一名參加。康橋校區另行通知

2、報名地點:校綜合樓三樓電腦機房。

備注:各年級電腦報名培訓結束時間為各班運動會報名截止時間。否則責任自負!

3.報名單上報后就不得更改(男子1500米、女子800米,1500米離比賽前一小時停止更

改),更改必須是報名單上無名的本班級同學。

4.比賽期間,某班如有運動員代表校、市、省比賽,可在運動會開幕前兩天到體育組替換

2013年黃驊市六年級期末統測方案

為貫徹素質教育精神,科學評估教學質量,根據《黃驊市教育局關于印發〈提高教育教學質量的工作機制〉的通知》(黃教通<2011>8號文)精神,本學期對六年級進行期末統測。本著“周密部署,嚴密組織,公正公平”的原則,采取全市統一時間、統一試題、統一要求、統一安排,各鄉鎮中心學校、市直中學負責具體組織實施的辦法。為確保統測成績的真實、客觀、公正,制定如下方案。

一、考生報名

1、各小學認真填寫《黃驊市2013年六年級學生成績登記表》,欄目依次為鄉鎮、考點、考場、考號、考生所在學校、班級、姓名、語文、數學、英語、科學、總分(有下劃線的不填),紙質表格由班主任和校長簽字后,加蓋學校公章后報鄉(鎮)中心學校;鄉(鎮)中心學校進行核實匯總,校長簽字并蓋章,于5月30日前報給教研室張建兵主任;同時將Excel文件合并后命名為“XX鄉(鎮)六年級學生成績登記表”,發至【注:考生所在學校一欄不要填寫教學點,要填寫其所在中心校。】

2、班主任、校長和鄉(鎮)中心學校校長為學生人數的責任人,要確保所有六年級學生都參加考試,各初中學校把小學六年級報名人數作為錄取七年級新生的主要依據。對故意隱瞞六年級學生人數的班主任、校長和鄉(鎮)中心學校校長通報批評,并將個人本的師德考核記為“不合格”。

3、確系特殊情況不能參加考試的,病假須出示縣級以上(含縣級)醫院證明,事假須學校出示證明,所有證明都必須有學生家長、校長和教辦室校長簽字同意,加蓋學校和鄉(鎮)中心學校公章,并將證明材料由鄉(鎮)中心學校于6月24日上午報到教研室張建兵主任存檔,過期不再受理。【注:缺考考生(證明材料齊全的請假)各科均按30分計入總分,無故缺考(截止到考試當天未見請假證明材料)的記為0分,卷面上出現污言穢語或有明顯反動言論的記為0分。】

4、所有考生信息與學籍表保持一致(包括姓名用字等)。

5、考生人員名單一經確定,不準變更。

二、組織辦法

1、考點設臵在各鄉鎮和市直中學。根據招生范圍,鄉鎮學校的六年級學生集中到本鄉鎮的相應中學,市直和民辦小學分別到三所市直中學考試。為防止停電,各考點學校應準備足夠數量的錄音機與干電池。另外,各小學定英語磁帶要保證至少兩個考場一盤磁帶,以備急需,磁帶在考試結束后交給考生所在學校。

2、將本考點來自不同學校的考生進行混編。各中學安排考場時,保證每個考場的考生來自各個不同的小學,交叉編排考號,除最后一個考場外,每場30人,單人單桌,一條龍順序排座位,貼好考號。各鄉(鎮)中心學校和市直中學將各考點的考生編號、考場安排于6月10日前發到郵箱jyjjyszjb@163.com,并將安排通知到相關小學。

3、試卷的領取、保管和封裝由各中學負責。各鄉鎮中心學校于6月24日下午3點前到教育局后面車庫取卷,分至各中學;市直中學于6月25日上午7點前取卷。對試卷要安排專人妥善保管,避免試題泄密。每場結束后,將學生答卷分考場用報紙當包頭紙封卷,統一交保管人員。全部考試結束后,各鄉(鎮)中心學校及市直中學把密封好的試卷統一交回教育局。

4、每考場兩名監考。市直中學監考由考點學校選派;鄉鎮中學交換監場。各鄉(鎮)中心學校根據教育局分配的監場教師名額,選派責任心強、處事公平的六年級教師到其他鄉(鎮)中學擔任主考,原鄉(鎮)中學再配一名監考。各單位于6月13日到郵箱查看2013年六年級統測輪換監考安排說明,并按說明落實。各接收鄉鎮及時與派出教師鄉鎮聯系,并將最終監考表回傳至派出教師鄉鎮。

5、考生持本人學籍表入場。各學校將學籍表提前發給考生,所有考生以學籍表作為入場憑證。主考老師要對考生學籍表逐一核對,發現替考者,馬上清出考場,該生所有科目考試成績一律記為0分。同時,對于該生和替考考生所在學校的校長、班主任老師及其他相關責任人的本師德考核記為“不合格”,造成惡劣影響者,進一步追究行政責任。

6、鄉鎮中學交換閱卷。各鄉(鎮)中心學校在考試當天把試卷交回教育局后,領取其他鄉鎮的試卷,組織本鄉(鎮)中學教師進行閱卷;市直中學直接閱本考點考生的試卷。閱卷教師要選派業務精通、責任心強、認真仔細的教師。閱卷登統后,各單位將《黃驊市2013年六年級學生成績登記表》按“學校”、“班級”排序,計算出每所小學各科成績的各“率”,填寫《黃驊市2013年六年級抽測成績匯總表》中的除各項得分、名次(紅色字體)外的其余欄目,于6月28日前將以上兩表發至教研室公共郵箱;同時把學生試卷和閱卷教師分工交到教研室。【注:成績匯總表中及格人數包括良好和優秀人數;良好人數包括優秀人數。】

7、考試和閱卷期間,考點中學校長為考點主任,教育局分派督導巡視員全程巡視,全部科目考試結束后護送鄉鎮中學考點的試卷回教育局。

8、集中查卷。發現考試成績與學生實際出入很大的,于7月1日以鄉(鎮)及市直小學為單位派一至兩名教育干部集中到后面車庫查卷。查卷中發現問題較大的,對相關閱卷單位給予通報批評。

四、工作制度

(一)監考規程

1、發卷前,向學生展示說明試卷袋密封情況,并當眾拆封,查考試卷與所考學科是否相同,如不相同,不能發卷,并馬上更換。

2、分卷以后,提醒學生核對試卷張數,要求學生在相應的位臵填寫好學校、班級、考號、姓名,并逐一查驗填寫情況。

3、將主監考教師姓名、本學科考試時間以及舉報電話(5313261;5332921)寫到黑板上。

4、每科考試結束,仔細查驗試卷數量,按照學生的考號順序、從小號到大號排序整理,確認試卷無遺漏、無倒裝、漏裝以后,用舊報紙做包頭紙密封試卷,再用訂書機在密封線處均勻地訂9個釘子,裝入試卷袋。

5、每科考完后,將密封的試卷送達領卷地點,交巡視員查驗。

6、考試中有異常情況,及時報告考點主任。

(二)監考職責

1、監考人員必須公正、公平,做到既嚴肅認真地維護考場紀律,又態度和藹地關懷考生。

2、嚴格按規定程序、時間組織考試,保證考試順利進行。

3、精力集中,忠于職守。不準離崗、串崗,不準抄題、做題,不檢查考生答題情況,不準提前或延長考試時間,不允許任何非考試人員進入考場,原則上不準吸煙和上廁所。

4、考試過程中,監考對試題內容不做任何解釋,但對試卷印刷不清等情況可當眾說明。

5、對違紀和舞弊行為進行及時有效的制止,認真填寫《考場報告單》(由考點學校提供),考試結束后交給考點主任,有問題的報告單在考試當天與試卷一同上交到教育局。

6、所有監考教師,不分單位,必須服從帶隊領導和巡視員的安排。

7、嚴格紀律。對在整個考試過程中玩忽職守、監考不力、造成不良后果、影響此項工作的教師,查實后由教育局在全市給予通報批評,并追究其所屬學校校長的責任。

(三)閱卷要求

1、按時到崗,守時守紀。閱卷過程中不要請假,不準閑談。

2、閱卷要公平公正,合理評分,特別是對于作文的評分,要考慮到小學生的實際寫作能力,避免以中學生的標準來評判。

3、按照分配的題目,與閱同一題的同志及時協商,標準一致。

4、如標準答案有誤時,須請示組長同意后才能更改。

5、在規定的地點閱卷,試卷不準帶出閱卷室,不能參與同本閱卷無關的事情。

6、責任明確。對閱卷人員進行分工,流水作業,閱卷人必須在自己所閱的相應題目旁的閱卷人一欄中簽字。

7、評卷一律使用紅筆,只能在卷面上畫寫評卷對錯標記和分數,記分數字要清晰,一律采用加分制,嚴禁涂改考生答案和成績。

8、不準私拆密封答卷冊,不準撬看密封線內考生信息。不準私自揭封頭的封條,如發現有倒裝、密封不嚴、卷面有標志等特殊情況,立即請示組長,妥善處理。

9、閱卷結束后,做好閱題復查和分數核查,杜絕漏判、漏登及計分錯誤,大家認同無誤、請示組長驗收后,閱卷工作結束。

(四)登統守則

1、全組人員在場,共同拆封。

2、試卷拆封后,原則上分數不能更改,如因分數加錯,經閱卷教師、組長、鄉鎮中心學校領導審定認同后,方可更改。

3、拆封后,首先將試卷分學科、按考號從上到下排好,以考場為單位進行登記。

4、電子表格登統。微機錄入時,一人讀分,一人錄入,一人監督,確保不出現任何錯誤。

5、全部登統完畢,復核無誤后,由各鄉(鎮)中心學校和市直學校按照要求將《黃驊市2013年六年級學生成績登記表》進行合盤、排序,計算并填寫《黃驊市2013年六年級抽測成績匯總表》,在要求的時間內將試卷返回,并將成績統計表發送到教研室公共郵箱hbhhjy@126.com。

6、對登統中玩忽職守、徇私舞弊、造成不良后果者,查實后,由教育局在全市給予通報批評,并追究其所在學校校長的責任。

五、注意事項

1、強化安全意識。各學校要確保師生安全,尤其是學生往返于考點中學,以及中午有考生在考點學校期間,一定要有小學校長、班主任組織帶領,不允許學生自行行動;考試期間也要有人照管,一旦有事,能立即找到考生學校領導。各鄉鎮派出的監場教師要有一名教育干部帶隊,對教師安全工作直接負責,原則上由派出單位統一組織到指定的監場學校,返回時由監場學校組織送回。如需租車,應查驗車況及駕駛員資質,保證萬無一失。

2、增強時間觀念。一方面,各小學組織本校學生按時參加考試;另一方面,派到其他鄉鎮的監場教師也要充分估計車速、路況、天氣等因素,至少提前半小時到達監場學校。中午不得飲酒,飯菜從簡,不得影響下午考試。

3、周密安排部署。這次活動規模較大,考點較多,各鄉鎮、學校要高度重視,認真組織,周密部署,做好每一個環節的工作,確保活動順利完成。特別需要注意的是,各類上報材料一定要嚴格把關,按時完成,發送電子文件要分清不同的郵箱。

4、嚴肅考風考紀。一方面,對于六年級學生,各小學應通知學生的考場考號,要求學生對號入座,自覺維護考場秩序,遵守考場紀律,保持室內外衛生。另一方面,對于各學校和教師,要正確對待這次統測,以真實的學生狀況、真實的學生成績,客觀地展現本校教學工作的面貌,凡有替考和不讓學生參加考試等舞弊行為的,一經查實,嚴肅處理。
