四、申請審批程序

1、申請學(xué)生應(yīng)于 2011年10月28日前向所在院系部遞交“申請自費(fèi)公派出國留學(xué)學(xué)生申請表”。學(xué)生要本著自愿的原則,根據(jù)自己家庭經(jīng)濟(jì)情況提出申請。申請表可從國際交流與合作處下載。學(xué)生申請項(xiàng)目時(shí),應(yīng)提供相應(yīng)的材料如課程成績單、由擔(dān)保人簽字的家庭經(jīng)濟(jì)條件證明書。

2、院系部對(duì)學(xué)生申請逐一進(jìn)行審查,并填寫“院系部推薦自費(fèi)公派出國留學(xué)生名單匯總表”。每個(gè)院系部按照推薦名額及綜合成績標(biāo)明排序,院系部蓋章后,于 2010年10月28日之前與有關(guān)材料(包括匯總表及其電子版、有負(fù)責(zé)人簽章的“申請自費(fèi)公派出國留學(xué)學(xué)生申請表”、申請學(xué)生的課程成績單)一并交至教務(wù)處。

3、教務(wù)處將通過審核的學(xué)生匯總名單(含紙質(zhì)材料和電子版)報(bào)國際

被推薦人的基本情況介紹。側(cè)重于個(gè)人的畢業(yè)時(shí)間、學(xué)校、所獲學(xué)位以及個(gè)人的專業(yè)經(jīng)歷。

推薦人對(duì)被推薦人的基本評(píng)價(jià)。側(cè)重于被推薦人的專業(yè)基礎(chǔ)、個(gè)性、特點(diǎn)、工作態(tài)度和在學(xué)術(shù)上的前途估計(jì)。若是推薦研究生,推薦人還需進(jìn)一步說明其深造學(xué)習(xí)的基礎(chǔ)和當(dāng)前所具備的研究能力。顯然,恰如其分地評(píng)價(jià)被推薦人的基礎(chǔ)、能力和前途,比言過其實(shí)的贊譽(yù)更令人情服,更具有實(shí)際意義。

推薦人可以著重介紹被推薦人曾經(jīng)獲得的獎(jiǎng)勵(lì),發(fā)表過的論文,參加過的重要學(xué)術(shù)會(huì)議,以及曾在學(xué)生組織或?qū)W術(shù)團(tuán)體中的任職等來支持自己的評(píng)價(jià)。

推薦人還必須清楚地表明被推薦人留學(xué)的身份是研究生還是訪問學(xué)者,專業(yè)領(lǐng)域和研究方向是什么。如果大學(xué)提供了現(xiàn)成的推薦表格,則必須按這類表格認(rèn)真逐項(xiàng)填寫。推薦表格中一般有學(xué)生綜合評(píng)估的一項(xiàng),即要求推薦人說明該生在所教的學(xué)生中應(yīng)列為前5%,10%,或25%等。這種評(píng)估是指教授的個(gè)人評(píng)價(jià),可以稍高一點(diǎn)。另外,推薦表格上常有許多難以填寫的項(xiàng)目,遇到這種情形可填i dont know,亦不致對(duì)該生的評(píng)價(jià)有何嚴(yán)重影響。

丙方(乙方擔(dān)保人):

住所:上海市路弄號(hào)室

復(fù)旦大學(xué)(甲方)與乙方和其擔(dān)保人丙方,就甲方派遣乙方因公出國交流學(xué)習(xí)并由丙方擔(dān)保事宜,簽訂如下協(xié)議:

第一條 甲方根據(jù)人才培養(yǎng)和事業(yè)發(fā)展的需要,因公派遣乙方出國交流學(xué)習(xí),乙方接受甲方的派遣。

第二條 甲方派遣乙方以____身份赴____________________________學(xué)校留學(xué),期限為_____個(gè)月(自出國之日起計(jì)算)。

第三條 甲方指導(dǎo)乙方制訂具體的留學(xué)計(jì)劃,甲方和乙方商定的留學(xué)內(nèi)容是:交流學(xué)習(xí)

第四條 甲方在乙方因公出國留學(xué)期間,指定復(fù)旦大學(xué)外事處對(duì)乙方給予必要的業(yè)務(wù)指導(dǎo)。

第五條 乙方保證按本協(xié)議書第二條、第三條的規(guī)定,在國外努力完成學(xué)業(yè),交流學(xué)習(xí)期滿后按期回國,并按期向甲方報(bào)到。

第六條 乙方在國外期間,保證遵守中國法律和中國對(duì)出國留學(xué)人員的規(guī)定,遵守所在國的法律和交流學(xué)校的規(guī)章制度。

第七條 甲方違約的,乙方可以單方解除本協(xié)議,請求甲方返還其因履行本協(xié)議項(xiàng)下義務(wù)發(fā)生轉(zhuǎn)移的財(cái)產(chǎn),并請求賠償直接經(jīng)濟(jì)損失。

乙方違約的,甲方可以單方解除本協(xié)議,乙方違反第五條約定的,甲方可以請求乙方支付人民幣_(tái)_____________ 圓整(____________元)的違約金。此外,甲方可以請求乙方賠償其直接經(jīng)濟(jì)損失。

第八條 在乙方違約的情況下,丙方就甲方向乙方提出的償付請求承擔(dān)連帶賠償責(zé)任。

第九條 本協(xié)議書自各方簽署之日起生效,簽字各方保證遵照執(zhí)行。有效期限至本協(xié)議書第五條規(guī)定的交流學(xué)習(xí)年限期滿時(shí)止。

第十條 各方就本協(xié)議項(xiàng)下約定內(nèi)容及相關(guān)事宜發(fā)生爭議的,應(yīng)當(dāng)友好協(xié)商,以促和解;協(xié)商不成的,任一方可以向上海市楊浦區(qū)人民法院提起訴訟。

第十一條其他約定:
