第二篇：辦理外國專家證申請材料所需清單

《外國專家證》申請材料清單

一、已取得《外國專家來華工作許可證》的：

1、外國專家來華工作許可證存根；

2、聘用單位申請報告；

3、打印《外國專家證申請表》一式兩份，加蓋單位公章；

4、有效護照及工作簽證復印件；

5、聘用合同復印件，須加蓋單位騎縫章；

6、衛生檢疫部門出具的健康證明書復印件；（中河中路230號浙江省國際旅行衛生保健中心 87852410）

7、2寸彩色免冠照片3張；

8、專家證有效期內的保單復印件；

9、隨行家屬須提供第4、6、7、8項材料，其中18周歲以下未成年人免第6項材料；

10、臨時住宿證明。

材料第5、6項如在辦理來華工作許可時已提交的，可以不再提交。

二、從境內其他單位轉聘的：

1、聘用單位申請報告（須注明申請人基本情況、申請事由、申請內容、申請期限）；

2、《外國專家證申請表》一式兩份，加蓋單位公章；

3、申請人與原聘用單位解除合同關系證明原件；

4、有效護照及簽證或居留許可復印件；

5、個人簡歷（包括學習和工作經歷）中外文各一份；

6、最高學歷證書或專業資格證明材料復印件及中文翻譯件；

7、經濟技術類聘用單位須提供營業執照復印件；

8、衛生檢疫部門出具的健康證明書復印件；

9、聘用合同復印件（須加蓋單位騎縫章）；

10、2吋彩色免冠照片3張。

11、專家證有效期內的保單復印件。

12、隨行家屬須提供第4、8、10、11項材料，其中18周歲以下未成年人免第8項材料；

13、如聘用專家原先有專家證的，需提供專家證注銷證明。

三、原單位續聘的：

1、聘請單位延期申請報告（須注明延期原因、申請延期的期限及專家資料變更說明）；

2、打印《外國專家證申請表》一式兩份，加蓋單位公章；

3、有效護照及簽證或居留許可復印件；

4、有效外國專家證；

5、聘用合同復印件（須加蓋單位騎縫章）；

6、2吋彩色免冠照片3張；

7、專家證有效期內的保單復印件；

8、隨行家屬須提供第3、7項材料。

四、《外國專家證》的注銷：

1、聘請單位注銷申請報告；

2、《外國專家證》原件。

第四篇：《外國專家證》辦理須知

辦理經濟技術類《外國專家證》須知

《外國專家證》是外國專家在華工作的身份證明，是辦理在華居留許可的必備證件，持《外國專家證》的專家享受國民待遇。《外國專家證》分經濟技術類和文教類兩種，專家證有效期限為一年，期滿后可延期。

一、受理對象

（一）為執行政府間、國際組織間協議、協定和中外經貿合同應聘來我市工作的外國籍專業技術和管理人員；

（二）應聘在我市外商投資企業中擔任副總經理以上職務，或享受同等待遇的外國籍高級專業技術或管理人員；

（三）應聘來我市從事經濟、技術、工程、貿易、金融、財會、稅務、旅游等工作，具有特殊專長、中國緊缺的外國籍專業技術或管理人員。

二、申報材料

（一）初次辦理

1、用人單位加蓋公章的申請公函，格式為：

關于為×××辦理《外國專家證的申請》

正文：介紹公司基本情況、聘請外國專家的姓名、年齡、國籍、護照號碼、簽證種類、擔任職務、聘請期限等基本信息。

單位名稱（公章）：

2、《外國專家證申請表》（可從張家港人才網下載，表格所有內容必須填寫完整，需用外文填寫的要用外文填寫）；

3、聘用協議或勞動合同復印件；（聘用協議需注明聘用起止日期，加蓋單位公章）

5、有效護照、簽證（居留許可）原件及復印件；

6、有效健康體檢證明（出入境檢疫部門出具）；

7、聘用專家簡歷（需加蓋單位公章）；

8、近期1寸免冠照片3張。

★ 有隨行家屬的，提供家屬的有效護照、簽證（居留許可）原件及復印件、近期一寸照片2張/人，并注明與專家本人的關系。

★ 以上材料均需一式3份。

（二）辦理延期

1．用人單位加蓋公章的申請公函；，2．《外國專家證申請表》；

3．聘用協議或勞動合同復印件；（聘用協議需注明聘用起止日期，加蓋單位公章）

4．有效護照、簽證（居留許可）原件及復印件。
