Muknapa Wiangkaew 為西南大學文學院泰國2+2項目商貿漢語班學生。

該生在校期間學習刻苦，學習態度端正，上課從不遲到早退，認真完成老師布置的各項學習任務，在上學期期末考試中各門課程均取得了優異成績，各科成績都達到了A等，名列班級前茅。

青島理工大學新生獎學金申請表

青島理工大學新生獎學金評選細則

第一章 總則

第一條 為吸引優秀生源報考我校，提高我校的社會知名度，鼓勵優秀學生奮發成才，扶持優秀困難學生順利完成學業，根據《青島理工大學獎學金管理辦法》，經研究決定設立青島理工大學新生獎學金。

第二條 青島理工大學新生獎學金是學校設立的針對入學新生的最高獎學金。

第二章 獎勵設定及評選范圍

第三條 新生獎學金發放金額為每學年50萬元。

第四條 新生獎學金根據當年具體報考情況評選，分兩部分：

一、全額獎學金，大致相當于大學本科階段的學費，15000元/人；

二、半額獎學金，大致相當于大學本科階段的學費的一半，7000元/人。

第五條 評選范圍：當年考入我校的本科新生。

第三章 評選條件

第六條 評選基本條件如下：

1、第一志愿報考我校；

2、高考成績特別優秀，至少高于所在省、市、自治區我校分批次最低錄取分數線40分以上（若我校在該省份有多批次錄取，則只針對最高錄取批次）；

3、高考成績優秀，至少高于所在省、市、自治區我校分批次最低錄取分數線20分以上，且家庭條件特別困難或遭遇重大天災人禍（須有當地縣級或以上民政部門蓋章的證明材料）；

4、高中期間曾獲得高層次獎勵，如：全國中學生學科奧林匹克競賽省賽區（競賽范圍以教育部2009年普通高等學校招收保送生辦法中公布的競賽名稱為準）一等獎以及全國決賽一、二、三等獎獲得者；全國青少年科技創新大賽（含全國青少年生物和環境科學實踐活動）或“明天小小科學家”獎勵活動或全國中小學電腦制作活動一、二等獎獲得者；在國際科學與工程大獎賽或國際環境科研項目奧林匹克競賽中獲獎者等。

以上四條中第1條為必要條件，第2、3、4條中只須滿足一條即可申報。

第四章 評審及頒獎

第七條

1、新生獎學金實行申報制，由考生本人直接向學校提出申請，并填寫申請書。申請分三個階段，第一階段：從高考成績公布后到報志愿結束前；第二階段：志愿報考結束后到本科錄取結束前；第三階段：若仍有余額，新生報到后可繼續申請。

2、由學生工作處會同招生就業委員會辦公室提出建議名單報校獎學金管理委員會主任審批。

3、招生就業委員會辦公室負責將學生獲獎通知隨新生錄取通知書一起發放，獎學金在學生報到后集中發放。

第八條 校獎學金管理委員會主任有權直接提名新生獎學金獲獎人選。

第九條 對未報到的獲獎新生取消獲獎資格，對弄虛作假的獲獎新生取消獲獎資格并追究責任。

第五章 附 則

第十條 本章程自公布之日起實施，由學生工作處負責解釋。
