中國人壽“學生平安保險”理賠所需資料及理賠事宜

一、辦理保險理賠所需資料

1.學生證復印件一份，身份證復印件兩份。

2.“學生平安保險卡”原件及復印件一份。

3.醫療費收據原件及復印件各一份

4.門診病歷復印件（意外事故門診歷上要有醫生開具體的詳細藥名）

5.住院病案首頁，主訴，檢查報告書，長期醫囑單復印件各一份（到入住醫院病案室索取）

6.出院小結，費用明細清單原件各一份（注。費用明細清單自己保留一份復印件）

7.意外事故證明材料。（意外傷病者提供，其他患者不需要此材料）

8.學生在校期間，如果需轉到外地治療的，必須征得保險公司客服中心同意。學生請與劉先生聯系。聯系電話：***

請備齊以上資料的同學，于每周二·五到校財務辦公室（行政樓401室）辦理保險卷宗（理賠委托書，授權書）簽字手續。

二、注意事項

1.學校網站財務處網頁“信息快遞”欄將及時公布獲賠付學生名單，請交齊資料的同學自交資料30日后上網查詢，查到自己名字后攜帶本人學生證到財務會計處核算科（行政樓404室）辦理相關事宜。

2.請各位同學妥善保管自己的“學生平安保險卡”（可復印一份備查）若有遺失，請直接與劉先生聯系，聯系電話：***.

第二篇：保險理賠所需準備資料及注意事項

保險理賠申請資料

1.門診病歷的原件和復印件各一份；發票原件訂在病歷原件上；

2.理賠申請書一份（需蓋章，填寫完整，若需退發票請在備注處寫明，并提供發票復印件一份粘貼在粘貼單上）；

3.賠款信息表二份（其中每一份信息表上都應粘貼上身份證復印件、中行或工行存折首頁的復印件，并填寫好相關資料，需蓋章的地方要蓋章）；

4.索賠資料粘貼單一份（請將學生證、發票復印件粘貼好）；

5.各種檢查的書面報告；

6.疾病診斷書（請將疾病診斷書貼在索賠資料粘貼單上）；

7.住院費用匯總表（不再退回，請自留復印件）。

注意：

＃申請理賠時，若是意外門診，則只需要準備以上1、2、3、4、5；若是住院醫療，則以上資料1、2、3、4、5、6、7全部需要。

＃學生只需準備資料，需要加蓋公章的地方由保險聯絡員負責。

注意：

1、借讀生無法提供戶口本需提供學校出具的證明（注明本人的姓名、戶口所在地及出生生年月日）然后加蓋學校公章。

2、二代身份證需提供復印件正反面。

If you would like to know services for settlement of claims of comprehensive insurance for people coming to China, please read this guide carefully.1、理賠程序

⑴ 保險事故發生后，理賠的規范程序:

直接向協議約定的中國平安人壽保險股份有限公司所屬分公司團險理賠部報案；

⑵ 理賠咨詢電話：

協議約定的中國平安人壽保險股份有限公司所屬分支機構電話或通過平安人壽北京分公司客戶服務咨詢及理賠報案電話010-66213388-69013直接查詢被保險人所在地的平安人壽分管機構電話及詳細地址；

1.Procedure of settling claims:

(1)Claim settling procedure against the occurrence of insured incidents:

Submit your claim to Group Insurance Division with affiliated companies under China Ping An Life Insurance Co., Ltd.as stipulated in the agreement.(2)Claim & consulting hotlines:

Please call the local branches under China Ping An Life Insurance Co., Ltd.or dial the service and claim consulting number: 010-66213388-69013 of Beijing Branch to inquire the numbers and specific address of the local branches as agreed in the contract directly.2、理賠應備文件：

① 團體一年定期壽險

A 被保險人護照復印件；

B 被保險人傷殘時需提供傷殘鑒定證明（由司法或勞動部門出具）；

C 被保險人死亡需提供死亡證明；

D 如發生交通事故，需出具公安交通部門的事故證明；

E 被保險人死亡時提供受益人身份證明復印件；

F 死亡賠付需提供受益人親筆簽字認可的保險分配方案的文件；

G 被保險人保險卡復印件；

2.Documents to be presented for settlement of claims:

①Group term life insurance of one year

A.Copy of passport of the insured;

B.Disability assay certificate in case the insured is disabled(provided by judicial or labor department);

C.Death certificate of the insured;

D.Proof of accident provided by the communications department of the police in case an traffic accident occurs;

E.Copy of the beneficiary’s identity proof in case the insured is dead;

F.Insurance distribution scheme signed and ratified by the beneficiary in death compensation;

G.Copy of the insurance card of the insured.②團體意外傷害附加醫療

A 被保險人護照復印件；

B 意外事故經過及證明（若是交通事故則需出具公安交通部門事故證明）；

C醫院診斷證明、病歷本以及門診收據、藥費處方、超聲波、CT、X光片、核磁等輔助檢查報告單；

②Annexed medical treatment to group accidental injury

A.Copy of passport of the insured;

B.Process of the accident and its proof(A proof of accident provided by the communications department of the police has to be presented in case a traffic accident occurs);

C.Diagnoses certificate and the medical records and medical examination receipt, diagnoses with medical charge, assist examination record of ultrasonic, CT, X-ray, nuclear and magnetic etc.D.Copy of the insurance card of the insured.③團體住院醫療

A 被保險人護照復印件；

B 意外事故經過及證明；

C 醫院診斷證明以及住院收據住院明細文件原件；

D 被保險人保險卡復印件；

③Group medical treatment in hospital

A.Copy of passport of the insured;

B.Process of the accident and its proof;

C.Original copy of diagnoses certificate and receipt and detailed documents for hospitalization;

D.Copy of the insurance card of the insured

④每次賠案文件中須附上被保險人或學校指定的銀行帳號,并本人簽字或學校蓋章。

④Account number of the insured or the school in designated bank and the signature or school’s seal is to be annexed to each of the claim settling documents.注意事項：Notes:

1、若一次保險事故分別在兩家（含）以上醫院就診，須出具相應醫院診斷證明書、病歷本等相關文件。

2、就診醫院為公立二級（含）以上醫療機構。

3、保險公司理賠完成后將加蓋本公司公章的醫藥費分割單同賠款一同寄至申請人，以報銷余下部分醫藥費。

4、護工費申請要求：

（1）被保險人個人申請所需材料：醫院出具的護工費收據；

（2）被保險人所在單位申請所需材料：醫院出具的護工費收據或單位開具的證明和護工本人的簽字并加蓋申請單位公章；

5、材料寄送地址：中國平安人壽當地分管機構規定的地址。

Notes:

1.Where in one insured incident, the insured has to be treated in two or more hospitals, diagnoses certificate from those aforesaid hospitals and the medical records and other documents in respect thereof shall be presented.2.The insured shall get treatment in hospitals which are public medical institutions and at least in second rank.3.After the insurance company settles a claim, dividable medical charge bill sealed by the insurance

company and the indemnity will be sent to the applicant so as to compensate the remaining medical charge.4.Application of Nursing Fee:

(1)Materials the insured is required to submit: Receipt of nursing fee issued by the hospital;

(2)Materials required from the applicant’s working unit: Receipt of nursing fee issued by the hospital or letter of identification with nurse’s signature and official seal of the unit of applicant;

5.Materials sent to: the appointed local branches subordinated to China Ping An Life Insurance.

平安公司學生保險理賠所需資料明細

一、意外門診：

1、保單復印件；

2、戶口增減頁、學生戶口分頁、家長戶口分頁及家長身份證復印件；

3、門診病歷、處方、費用清單、門診發票原件；

4、照片、CT等診療報告單；

5、家長銀行存折或銀行卡復印件。

二、住院醫療：

1、保單復印件；

2、戶口增減頁、學生戶口分頁、家長戶口分頁及家長身份證復印件；

3、出院證、出院記錄、住院費用總清單、住院發票原件；（如有合作醫療先行報銷的，并且發票為復印件的，請提供醫院發票復印件并加蓋醫療報銷機構公章，和合作醫療報銷證明原件）

4、住院病歷復印件；

5、家長銀行存折或銀行卡復印件。

三、身故責任：

1、保單復印件；

2、戶口增減頁、學生戶口分頁、家長戶口分頁及家長身份證復印件；

3、戶口注銷證明復印件；

4、意外事故說明或醫學死亡證明復印件；

5、火化證復印件；

6、由村、居委會出具蓋章并由當地派出所蓋章證明的學生與家長（父母）的關系證明原件；

7、父親、母親的銀行存折及銀卡復印件；

8、如交通意外需提供交警部門出具的交通事故認定書。
