如何填寫簽證中的申請表呢？

在邀請外國人來華簽證申報表中：

“被邀請人情況”欄必須打印。被邀請人“國籍”和下面的“簽證地”須用英語填寫。

外商系首次來華時，“來華事由”欄要詳細填寫有關該公司的性質(zhì)、經(jīng)營范圍等情況，以及如何與該公司結識的，本次來訪的主要目的等內(nèi)容。對于辦理二次和半年多次往返的申報，“來華事由”欄要寫明最近半年來華共有多少次和本次要申請多次往返的充分理由。

“邀請單位擔保意見”內(nèi)容大致如下：

茲有我公司****國客商******（護照號碼為******）于200*年*月來義烏進行合法正當?shù)纳虅栈顒樱Ａ魰r間為**天，我公司擔保客商在此期間不產(chǎn)生違反法律的行為。

“在華停留天數(shù)”最長為25天。

對于“簽證地”與“國籍”不一致的客商，要求在每一份材料中附上該客商在“簽證地”之國的居留證復印件，并注明“這是某某在某地的居留證。”，由邀請單位蓋上公章。

辦理多次往返的，要求企業(yè)前半年已邀請該客商五、六次來華，并在每一份材料中附上該客商的每次來華簽證記錄、海關出入境記錄和貿(mào)易額憑據(jù)。

“領導簽名”欄，企業(yè)法人代表或其委托領導須親筆簽名（企業(yè)經(jīng)辦人不可擅自代簽，也不可以蓋領導簽字章），后蓋公章。

“聯(lián)系人”要填寫企業(yè)經(jīng)辦人自己的聯(lián)系方法（為不耽擱您的寶貴時間，最好寫上手機號），而且要表中所寫的“聯(lián)系人”本人去金華取簽證通知，不可委托快遞公司取。

護照的復印件必須包含如下四項內(nèi)容：

1、外商的姓名全稱；

2、國籍；

3、護照的有效期；

4、護照的號碼；

如果有身份信息，必須是商人身份，不可是與商務無關的身份。

雙方購銷合同要求是由外商簽字、我方企業(yè)負責人簽字并加蓋公章后生效的合同或售貨確認書等材料的復印件，以證明雙方之間的經(jīng)貿(mào)關系。

各種附件不可以使用感應傳真紙，感應傳真紙上的內(nèi)容必須經(jīng)復印后，方可使用。

各企業(yè)邀請外商來華，由各企業(yè)外事專辦員到義烏外僑辦辦理。

外國人來華邀請函

一、申請邀請函的中國企業(yè)資格

1、在我省依法登記的法人企業(yè)（注冊資金在50萬元人民幣以上）（以下簡稱邀請單位），可申辦邀請與本單位有實質(zhì)業(yè)務的外國人以商務（f字）簽證入境。

2、注冊資金不夠的單位，必須出具與有申辦資格單位的合同或擔保函；沒有出口權的單位，必須出具與有出口權單位的合作合同。

注：以上期限以合同期限為準。

二、備案，初次申辦商務(f字)簽證通知表需提交的材料（一式兩份）

1、如實填寫：“邀請單位申辦《被授權單位簽證通知表》備案表” [附件:申請單位備案表.doc]

2、企業(yè)法人身份證復印件或護照復印件（簽名、辦公電話及手機號、加蓋企業(yè)公章）；

3、專辦員（必須是邀請單位員工）身份證復印件（簽名、辦公電話及手機號、加蓋企業(yè)公章）；

4、邀請單位企業(yè)營業(yè)執(zhí)照副本復印件（上年度已通過年審、加蓋企業(yè)公章）；

三、審批流程

首先，根據(jù)外商來華目的，合資格的企業(yè)將申請資料送到順德貿(mào)促會辦理；然后，經(jīng)順德貿(mào)促會初審后，再送到省貿(mào)促會審批；最后，省貿(mào)促會把邀請函的正本寄回順德貿(mào)促會，由順德貿(mào)促會通知并轉交申請企業(yè)。

四、申請資料

須經(jīng)常來華（三個月以內(nèi)兩次以上進入中國境內(nèi)進行商務活動的）的外籍商人，申請資料必須提供最后一次來華的出入境簽證記錄。不予提供者，我會不接受申請。

如被邀請人屬首次來華，外國公司出具的在職證明掃描件（須用外國公司信箋紙出具該英文證明，內(nèi)容包括該外國商人在該公司所任職務、護照號、來華目的、出入境時間、旅費是否足夠等，加蓋外國公司公章及相關負責人簽名）； 邀請單位出具與外國公司商務往來證明（有效經(jīng)貿(mào)合同，真實的商務往來email原件的打印件）。（2）用新護照申請時，需同時提供舊護照復印頁及舊護照來華簽證記錄。（3）從2006年9月12日起，已停辦理尼日利亞籍商人來華邀請，何時恢復辦理，請留意網(wǎng)上有關通知。
