在昨天得知XX家園燃氣事故后，在慶幸沒有發(fā)生人員傷亡的同時，也對自己工作的失職感到深深的自責，我做為安全監(jiān)察部的部長，對此有不可推卸的責任。

上任后，自己在開展各項工作的時候，忽視工作內容中包含的責任與義務。在完成領導安排的《值班管理制度》任務后，沒有針對制度中的各項要求進行檢查，疏于監(jiān)管。對此，我誠心檢討，并接受公司的懲罰。

此次安全管理的疏忽已給我敲響了警鐘，我將引以為戒，并以此為契機，深刻檢查我們安全監(jiān)察部在安全管理中的薄弱環(huán)節(jié)，端正工作態(tài)度，對內加強公司各項工作的安全監(jiān)管，橫向開展各項檢查工作，縱向加大檢查力度，堅決執(zhí)行各項獎懲制度，對外加大安全宣傳力度，在轄區(qū)內營造安全用氣氛圍，樹立公司注重安全，服務優(yōu)質的良好形象。向公司、向同事、向社會展現(xiàn)一個安全管理新局面。

201X年X月X點左右，我公司轄區(qū)內盈南家園小區(qū)的燃氣設施被不法分子非法盜竊、破壞，造成天然氣泄漏。因發(fā)現(xiàn)早，地處戶外，未發(fā)生燃氣事故。

對此，我公司高度重視，緊急召開安全領導小組會議，會議部署管網運行部加強管網、庭院巡檢力度；客戶服務部聯(lián)系轄區(qū)內各小區(qū)物業(yè)，要求物業(yè)支持并協(xié)助我公司的安全工作；安全監(jiān)察部聯(lián)合客戶服務部，對轄區(qū)內部分小區(qū)進行以“燃氣連著你我他，安全用氣靠大家”為主題的安全宣傳。

在使用您的燃氣表前，請詳細閱讀以下信息：

CG-Z-JRB2.5IC卡家用膜式燃氣表技術指標符合國家標準GB/T6968-1997及建設部CJ/T112-2000行業(yè)標準的要求，采用家用燃氣表JJG577-2005檢定規(guī)程，可適用于城市管道煤氣、天然氣、石油液化氣、沼氣等燃氣氣源的自動計量和收費控制。該燃氣表有兩套顯示系統(tǒng)，分別為機械字輪顯示器和以微電腦為控制器的液晶顯示器/。In the use of your gas meter, please read the following information:

2、關閥：您的表具系統(tǒng)具備智能判別能力，如果表具上的“存（余）量”小于等于0 m3、或者出現(xiàn)“故障：干擾”、“換電池”字樣時，若已打開的閥門此時將被關閉，液晶上原有的“開閥”字樣將會消失。如果您希望打開閥門以便使用，那么您需要確認是由于“存（余）量”不足造成的不開閥，還是由于電池電量不足、故障造成的不開閥，然后根據(jù)您所獲得的信息進行下一步操作。

3、充值操作：當您的表具的液晶上顯示“請充值”字樣時，說明您的表具的“存（余）量”已小于3 m3，此時，您需要進行充值操作；首先，帶上您的用戶IC卡到燃氣運營商的服務點進行充值操作；然后，再將您的用戶IC卡按正確的方向插入表具的IC卡座，表具讀入數(shù)據(jù)后，會在液晶上按照“購入”—＞“總量”—＞“存量”—＞“用量”的步驟進行顯示您表具上的數(shù)據(jù)。

4、換電池操作：當表具的電池電壓過低時，表具的液晶將會顯示“換電池”，同時閥門將會關閉，此時，您需要將表具中的舊電池取出，然后裝上您預先準備發(fā)好的新堿性電池，待系統(tǒng)重新啟動即可。

6、當您的表具的“存（余）量”大于3 m3且表具沒有電池電壓過低時，表具的液晶顯示器將會關閉，以節(jié)省您的表具系統(tǒng)的電力。

7、使用壽命的限制：您的燃氣表的使用壽命受以下條件約束，當滿足其中之一，就表明您的燃氣表的使用壽命已到期，請聯(lián)系您的燃氣運營商予以更換： a 當使用年限已超過10年時，您的燃氣表將詞永久性關閉；

b 當您的燃氣表的“總量”大于999999.999 m3時，您的燃氣表將永久性關閉；

c 當您的燃氣表的“存（余）量”與“購入”之和大于999999.99 m3時，您的燃氣表將不再讀卡。其中，正式啟用是以您第一次將您的用戶IC卡插入您的燃氣表的日期為啟用時間的，因而，燃氣表安裝完成后，未正式啟用的時間是不包含在內的。

333故障診斷與排除 “故障：干擾”：

引發(fā)此故障的原因有兩種：

1、在燃氣表具附近存在強磁場源，請移除磁場源或改變表具的裝機位置，避免磁場的干擾；

2、由于運輸、摔打、振動造成內部的傳感器損壞，請聯(lián)系您當?shù)氐娜細膺\營商，以排除故障。“故障：計量”：

請與當?shù)氐娜細膺\營商聯(lián)系，讓運營商派員前來排除故障。“故障：非法”：

造成非法故障的原因，通常是因為您對表具本身作了一些非法的操作，例如攻擊、拆卸等等，此時，您應與當?shù)厝細膺\營商聯(lián)系，以便再付費更換表具。而出現(xiàn)此故障的表具是無法進行維修的，也就是說，只要表具出現(xiàn)這個故障，則表具已保護性永久損壞。

當表具出現(xiàn)任何故障，請您不要擅自進行維修，更不要破壞表具結構與封裝，以避免引起法律糾紛。

您的燃氣表是一臺精密計量設備，在使用過程中，請注意以下事項：

1、您的燃氣表的計量介質可以是天然氣、液化石油氣、煤氣、氫氣，請不要使用除這些媒介以外的其他氣源；

2、您的燃氣表應安裝在無腐蝕、無輻射、無雨水的環(huán)境中；

3、在燃氣表安裝時，應避免雜物進入管道、避免使用流體進行檢漏、嚴禁明火檢漏；

4、燃氣表在運輸時，應杜絕摔打、劇烈振動、上下倒置等操作，否則將影響燃氣表計量精度，以避免給您造成不必要的損失；

5、請使用規(guī)定的電源類型，否則將有可能造成燃氣表的永久損壞；

6、請不要在超出額定最大流量的條件下使用，否則可能造成計量偏差。

關于12月15日晚錯誤的出現(xiàn),完全是由于本人在工作上和心理上粗心大意造成的.從而給部門,課室,甚至公司造成了不必要的損失,成品的交期也被延誤,也增加了同事們的工作量!所以錯誤的教訓是沉重的,本人要從沉重的教訓中,總結教訓,汲取經驗...經過這次的錯誤,本人已經深刻認識到工作必須認認真真,一絲而不茍!要有嚴謹?shù)墓ぷ鲬B(tài)度,一刻也不能松懈,同時要不斷地提升自己的技能,完善自己,發(fā)揮自身的價值,為課室,公司貢獻一份力量!...在今后的工作中,望上級領導予以監(jiān)督及檢查.檢討人:顏向群

第五篇：事故檢討書

檢討書

尊敬的各位領導：

2013年4月30日下午16：00時左右，生產車間轉大班，全廠機電人員在檢修，C D拌和樓機修黃儒聚在D車間1#拌和機，檢修拌和機的底圍板，黃儒聚站在拌和機的下料斗中，雖然做到“五步驟”：“拉閘、驗電、掛牌、上鎖、設立監(jiān)護人”（有兩人共同檢修），但未做到“關汽閥、撥汽管、取插銷”，黃儒聚在維修拌和機底板時用鐵錘進行敲擊，導致汽管開關因鐵錘敲擊振動，而自動擊發(fā)，閉合了拌和機底門，底門夾住了機修黃儒聚的胸部，經送往松江中心醫(yī)院搶救無效死亡。

此次事故，給死者家屬造成了巨大的悲痛，給公司同事帶來了巨大的痛苦和心理陰影。同時，也給公司的形象造成了極其不良的影響。

事故雖然過去已經兩個多月，但我至今依然感到非常的痛苦和內疚。在這段時間里，我認真地進行了思考和悔悟。作為車間管理人員，我沒有認真執(zhí)行公司的安全規(guī)章制度和公司領導的工作指令,以及深入細致地做好安全監(jiān)督工作，對轉班檢修工作中的不安全狀態(tài)沒有進行細致、深入、全面的排查。在檢修過程中，對危險源的辨識及預測能力不夠。

對于此次事故，我認為責任有以下兩點：

1、作為公司管理人員，在轉班檢修過程中沒有進行有效的監(jiān)督，員工有沒有按規(guī)操作，管理人員監(jiān)督不到位，是這次工亡事故發(fā)生重要原因，作為車間負責人，我負現(xiàn)場管理責任；

2、機修黃儒聚作為多年的老員工，沒有按照公司安全規(guī)章制度進行檢修，盲目蠻干，存有僥幸心理，貪圖方便，明知故犯才導致事故的發(fā)生，是本次工亡事故發(fā)生的最主要原因。

經驗及整改措施如下：

1、必須加大現(xiàn)場監(jiān)督頻次，加大對違章指揮、違章作業(yè)人員的處罰力度；對檢修現(xiàn)場的監(jiān)督，不能只局限在行為管理，在安全教育方面，應對機修人員工作過程，結合檢修過程進行監(jiān)督與控制；

2、對每一處維修的機修人員的情況，做到及時跟蹤監(jiān)督；制定出解決問題的辦法和預防問題的措施，樹立“嚴是愛、松是害”的思想觀念。杜絕僥幸心理。

3、如何加強員工的安全教育，提高員工的安全防范意識，是搞好安全工作的一個重要保證，安全教育只能起到最基本的預防作用，在具體維修檢修過程中，如何讓員工提高自身的安全意識，并加以有效的監(jiān)督，是我們急需解決的問題根源。

此次工亡事故，我辜負了公司領導的教導與期望。本人認真總結安全管理的不足與教訓，今后無論在哪個崗位上，都要加倍的努力工作以回報公司。

總之，此次事故對本人的教訓是深刻的。我深知，任何道歉也不能挽回已發(fā)生的工亡事故，任何懺悔檢討在工亡事故面前，是顯得如此的蒼白與無力。在此，我只能說，我愿意接受公司領導對我的處理，并愿意以自己為反面教材，以此為鑒，遠離安全事故，以我為鑒，警示后人！
